Introduction
Osteoporosis is a degenerative skeletal disorder characterized by an increase in bone fragility and fracture risk due to a loss of bone mass and microarchitectural deterioration. The most serious clinical expression of osteoporosis is fracture. Despite the availability of a number of drugs capable of reducing fracture risk by over 50%, the use of drugs to prevent bone loss remains inadequate.

Bisphosphonates will remain the gold-standard approach to osteoporosis treatment due to familiarity among prescribers, genericization of leading brands and innovations in dosing among novel products. Other antiresorptive agents will significant potential be limited by high costs, safety concerns and inadequate commercial development plans.

The osteoporosis pipeline is top-heavy, with 50% of drug candidates in Phase III or higher. Investment is evident across therapeutic classes, particularly in improving peptide delivery methods, although anabolic therapies are dominated by early-Phase PTH drugs and comprise only one-fifth of total pipeline candidates.
