Coordinating a Virtual Medical Home in Your Community: Lessons from the Iowa Collaborative Safety Net Provider Network, a 45-minute webinar on September 23, 2010

After identifying a need in the state of Iowa for access to affordable prescription drugs and access to specialists for underserved populations, as well as medical home development for the safety net providers in the state, the Iowa/Nebraska Primary Care Association was awarded state funding to create and manage a provider network to bring together all of the community resources needed to make a medical home possible.

During Coordinating a Virtual Medical Home in Your Community: Lessons from the Iowa Collaborative Provider Network, a 45-minute webinar now available as a CD-ROM (for computer play), via On Demand Web access or as a training DVD, Sarah Dixon Gale, lead contract manager, Iowa/Nebraska Primary Care Association and Michelle Stephan, chief executive officer, Siouxland Community Health Center, shared how the primary care providers in this community work with other local, community-based organizations, as part of a virtual medical home, to improve access to and the quality of care.

- Developing a community utility approach to medical home development, where local public health and social service organizations can more easily provide support to primary care providers working to become medical homes;
- How minimal amounts of funding can encourage local entities to come together to begin to develop virtual or community-based medical homes;
- Creating formal partnerships between primary care providers and local hospitals to increase the range of services available to underserved patients and decrease the amount of uncompensated care and inappropriate use of the emergency room;
- Using a centralized care coordinator or patient navigator to assist patients in securing necessary resources that may not be available within a primary care provider office; and
- A case study on how a local primary care provider works with two local hospitals to coordinate the care of patients, including specialist care.
