Accidents are one type of trigger to set the process in motion.
An accident happens; you investigate; you discover causes and, perhaps, some hazards of which you were unaware. You correct these and time passes. If your investigation truly discovered the underlying causes and the proper corrections were made, perhaps a long time passes, and you may have prevented that same accident or a similar one from occurring again. Then there is another accident, perhaps a very different type, and the whole process is repeated. Again, the goal is to discover unrecognized hazards, correct them, and create a safer workplace for you and your co-workers.
