Transmucosal routes of drug delivery (i.e., the mucosal linings of the nasal, rectal, vaginal, ocular, and oral cavity) are well established. For example, the oral transmucosal route has been used for many years to provide rapid blood nitrate levels for the treatment of angina pectoris. They offer distinct advantages over other routes of administration including:

- Well suited for both systemic and localised conditions.
- Bypass of first-pass metabolism by the liver.
- Improved drug absorption (the stratum corneum epidermidis, the major barrier to absorption across the skin, is absent from the mucosal linings).
- Rapid onset of action.
- Sustained delivery of products over a pre-determined period of time.
- Noninvasive and convenient administration.

The first large-selling medicine to be adapted to transmucosal delivery was a lozenge formulation of fentanyl citrate known as Actiq. Actiq’s maker Cephalon subsequently produced a generic version of the medicine and in 2007, the two transmucosal fentanyl citrate preparations recorded sales of Euro 297 million. Other developers of transmucosal delivery systems are commercialising technologies to treat such diverse conditions as angina pectoris, breakthrough cancer pain, diabetes, erectile dysfunction, hypogonadism, insomnia, migraine, moderate to severe chronic pain, nausea/vomiting, opioid dependence, osteoporosis, overactive bladder, pre-procedure anxiety and Sjögren's syndrome. It is no surprise to learn then that the transmucosal drug delivery market was estimated to be worth Euro 1.48 billion in 2010.
