Cardiovascular disease is the most common cause of death in people with diabetes. Several factors have reawakened an interest in the assessment and treatment of heart disease. In particular, the DIGAMI study on the use of intravenous insulin infusion at the time of myocardial infarction has stimulated many discussion papers on the best treatment of MI in the diabetic patient. The UKPDS has shown that treatment of Type 2 diabetes does not reduce cardiovascular end-points significantly, but that aggressive treatment of blood pressure can reduce many cardiovascular end-points. In addition, sub-group analysis from several other studies has shown that treatment with statins in people with diabetes will also reduce cardiovascular events. Areas of controversy (treatment of diabetes post-MI with insulin, use of heart transplantation, bias against treatment in people with diabetes) are discussed in detail.
