Maximizing the Results of Your Disease Management Programs Through Community-Based Resources
|
Description: |
Maximizing the Results of Your Disease Management Programs Through Community-Based Resources, a June 21, 2006 audio conference on CD-ROM, examines how disease management programs can improve their effectiveness through partnerships with community-based organizations.
Community-based partnerships are a critical element in the Chronic Care Model developed by Dr. Ed Wagner, the director of Improving Chronic Illness Care. Partnerships can be formed with state programs, local agencies, schools, faith organizations, businesses and clubs.
During this 90-minute audio conference, your expert panel of speakers, Michelle Brooks, RN, MSN, administrator, regional health plans for Pitt County Memorial Hospital, University Health Systems of Eastern Carolina and Judith Szilagyi-Neary, clinical care manager at Ovations, a United Healthcare Company, provide inside information on how to choose a community partner, structure these partnerships and evaluate the impact partnerships can have on the effectiveness of your program.
You will get details on:
- Identifying what organizations you can partner with; - Structuring the partnership to maximum benefit; - Overcoming the challenges associated with community partnerships; - Maximizing the results of disease management programs through community resources; - Examples and benefits of community partnerships in place; and - Special considerations for the Medicare and Medicaid populations.
You can "attend" this program right in your office and enjoy significant savings – no travel time or hassle; no hotel expenses. It’s so convenient! Invite your staff members to listen in.
Who will benefit from this audio conference?
CEOs, medical directors, disease management directors, managers and coordinators, health plan executives, care management nurses, business development and strategic planning directors.
