Since Hurricane Katrina devastated the New Orleans region, the healthcare industry has been reshuffled and reshaped with several new hospitals rising to replace those closed in the storm’s wake. Federal and state money poured into the market, but it remains an extremely challenging financial environment for community hospitals. Just as some have started to stabilize, they are losing their disaster relief funding this year as well as having their Medicaid reimbursements cut. While the recession has affected the city, New Orleans unemployment still remains lower than in many U.S. cities, but that hasn’t lessened the weight that health systems and physicians feel from serving the half of the city’s population that’s either uninsured or enrolled in Medicaid. Continued Medicaid will exacerbate the problem and lead hospitals to press harder for higher reimbursements from commercial payors, evidenced by the hard negotiations between the region’s largest insurer and second-largest hospital.

Questions Answered in this Report

The largest health system in the market, Ochsner Health System, now corners nearly one-third of the hospital market in terms of inpatient discharges. Which health system recently purchased another hospital in the market? What kind of reimbursement rate increase does East Jefferson General Hospital say it needs from the area’s largest insurer to overcome its charity care debt? Which areas of the market are still ripe for expansion?

New Orleans-area physicians are not overly consolidated but have formed into several large academic groups. How much have physicians seen their Medicaid reimbursements cut? What happened that helped physicians pay less in the state for their medical malpractice coverage? Does New Orleans still have a physician shortage?

Health plans in the market are only moderately consolidated with Blue Cross and Blue Shield of Louisiana accounting for nearly a quarter of enrollment. What new reimbursement structure has BC/BS of Louisiana proposed that could shift more risk to area hospitals? Which insurer recently gained thousands of lives from the market’s other two top insurers by picking up a state contract? What new mandates were passed for health plans in the 2010 legislative session?
