Omaha continues to fare better than most U.S. cities, with one of the more robust metro economies, low unemployment and comparatively low uncompensated healthcare costs. Health systems and physicians, which already have the market heft to pass on the costs of continual expansions, are consolidating their power by creating large accountable care organizations. If their efforts at forming narrow networks for at-risk contracting are successful, it could drastically change the healthcare landscape locally. But conservative Omaha is not known for being an early adopter of reforms, and smaller networks or reimbursements that reward quality rather than volume will not happen overnight. In any case, payers will continue to be at a disadvantage, with employers lacking any vehicle such as a health coalition through which to consolidate their own power.
