Dawn Bazarko, senior vice president of clinical innovations at United Healthcare, and Dr. Lonnie E. Fuller, medical director for the Pennsylvania Medicaid ACCESS Plus PCCM-DM Program, provided the inside details on:
- Coordinating care effectively in medical homes;
- Creating marketing and communication strategies for medical homes;
- Evaluating quality and cost outcomes of medical homes;
- Coordinating care across patient conditions, services and settings over time;
- Encouraging patients to engage in self-management and providing them with the resources to do so;

Dr. Lonnie E. Fuller Jr., is currently the medical director for the Pennsylvania Medicaid ACCESS Plus PCCM-DM Program. Dr. Fuller has dedicated his career to uncovering and diffusing new concepts and practices throughout the medical community.
He previously served as the medical director of Hamilton Health Center in Harrisburg, Penn., a federally qualified community health center serving the un- and underinsured of central Pennsylvania. Dr. Fuller worked to spread a collaborative sense of ownership among staff to facilitate the improvement in the lives of those less fortunate.
Dr. Fuller also served as ProMetrics Consulting’s chief medical officer. In this role, he provided ProMetrics' incubator clients with an expert medical perspective, experience writing and analyzing business proposals for the medical community, and ensured the implementation of strategy for new medical services. Dr. Fuller also provided clinical and business consultation to ProMetrics’ traditional healthcare clients.
In a previous role at Chestnut Hill HealthCare, an integrated healthcare organization consisting of an acute hospital, a rehabilitation hospital and several ambulatory facilities, Dr. Fuller championed a "functional links" strategy, culminating with physicians accessing hospital lab and x-ray reports via online access. He also co-developed a tiered marketing plan for the hospital and built a database to track utilization of inpatient and outpatient services. He authored a paper entitled, "Market-Based Approach to Information Management Development" and developed plans to build Chestnut Hill Medical Associates, a not-for-profit physician employment model and recruited physicians.
Dr. Fuller has served on several influential boards, including: Clinical Regional Advisory Network, Harrisburg City Health Board, Board of Health-City of Philadelphia, Hill IPA, Chestnut Hill HealthCare System Board, Freedom Theatre of Philadelphia and the Student National Medical Association. Dr. Fuller is a member of both the American College of Physicians and American College of Physician Executives.
Dr. Fuller completed his internship and residency in Internal Medicine at the Medical College of Pennsylvania, Philadelphia, Pa. He received his medical degree from University of Texas Medical Branch, Galveston, Texas and received his bachelor's degree in physics at Franklin & Marshall College in Lancaster, Pa.
Dawn Bazarko is the senior vice president of clinical innovation at UnitedHealthcare, leading the primary care, emergency care, and integrated behavioral health programs serving over 10 million members nationwide. Her responsibilities include development and oversight of national programs focused on the evaluation and improvement of quality, safety, and healthcare affordability. These efforts include the deployment of process and health information technology solutions aimed at improving the quality, usability and efficiency of healthcare. As a published author in peer-reviewed journals, her areas of research focus include the study of health information technology and clinical quality improvement.
Bazarko also leads the development and execution of UnitedHealthcare’s patient centered medical home demonstration pilot. This pilot offers the opportunity to test a new model of primary care delivery and an alternative method of reimbursement that recognizes physicians for the delivery of better coordinated, comprehensive care management.
