Objectives: To periodically track changes in usage of Revlimid by surveying a set of Oncologists and Hematologists regarding their current and future practices in treatment of patient with multiple myeloma (MM) and myelodysplastic syndrome (MDS).

Specifically to:

- Determine the current market share held by Revlimid and other medications for MDS/MM patients.
- Evaluate Revlimid based on physician and patient opinions.
- Anticipate the future use and changes from current use of Revlimid.
- Reveal side effects and cost issues Revlimid patients experience.

Inclusion Criteria and Respondent Demographics:

- Board-certified or eligible Oncologist or Hematologist
- In practice between 2 and 30 years
- Spends at least 75% of professional time in clinical practice
- Have prescribed Revlimid for at least 10 MM and/or MDS patients
