Luke Hansen, MD, MHS, is the Vice President and Chief Medical Officer for Population Health for AMITA Health, a nine-hospital $2 Billion health system serving the greater Chicago area. In this role he provides clinical direction for the system’s value-based care and risk-based contracting portfolios including Medicare and commercial ACOs and the 1,600 physician AMITA Health Clinically Integrated Network.
Dr. Hansen is a practicing internist with a focus in hospital medicine as well as longitudinal care of high-cost and medically complex patients. He is the recipient of the 2011 Society of Hospital Medicine Young Investigator Award as well as funding from the National Institutes of Health and is an expert on hospital-based care transitions.
Dr. Hansen is a hospital site mentor for the Society of Hospital Medicine’s Project BOOST (Better Outcomes through Optimizing Safe Transitions) and in this role has coached over two dozen hospitals on implementation of best practices to improve care transitions. He is a fellow in the American College of Physicians and a former member of the College’s Health & Public Policy and Education Committees.
