The neurosurgical microscope market is a mature market and the vast majority of units sold are intended to be replacements for existing systems.

Though microscopes have a limited lifespan, most of the time they are purchased due to technological advancements before needing to be replaced. However, there is still low growth for units sold. There has been some competition from neuroendoscopes as they become more frequently used in neurosurgery but for now the surgical microscope remains the most frequently used optical tool in neurosurgery with the endoscope primarily used for assistance with certain procedures.

Leica has produced a 3D heads-up system which saves surgeons from having to look through the oculars of the microscope for long portions of the day.
