Bipolar instruments have two electrodes so that the current is passed from one electrode through the grasped tissue to the other electrode to complete the circuit. Bipolar energy can still spread to the adjacent tissue if the instrument is active for a prolonged period of time, or used with excess force; however, this occurs with much less frequency than with monopolar devices. Traditional bipolar instruments are almost always reusable.
Another direct energy device that has become very popular is the advanced bipolar device, which also has a vessel sealing function. These instruments are disposable devices that use the heat conducted from the probes to close off small or large vessels by “pulverizing” the surrounding tissue in order to stop bleeding.

Direct energy devices belong to a group of instruments in the electrosurgery sector that conduct high frequency alternating current through a patient’s body to generate heat energy. The circuit is composed of an electrosurgical generator, an active electrode, a patient and a patient return electrode. Commonly, these instruments can be differentiated as monopolar or bipolar and can be used for cutting, coagulating, desiccating or fulgurating tissue.
