Rising Cancer and Chronic-Disease Diagnostic Workloads

Cancer incidence continues to create demand for histological and cytological examinations across the cellular pathology market. IARC recorded 20.6 million new cancer diagnoses in 2024, and lung, female breast, and colorectal cancers were the largest incidence categories. Annual cancer incidence is projected to reach 35 million cases by 2050. Tissue assessment also supports diagnosis for rheumatoid arthritis, autoimmune hepatitis, primary biliary cholangitis, and IgG4-related disease. Higher screening coverage for these conditions can add slide volumes in histology and immunohistochemistry laboratories. National prevention programs that incorporate early detection can make pathology capacity a lasting part of public-health delivery.

Precision Oncology and Companion-Diagnostic Adoption

Companion diagnostics create recurring testing needs because treatment protocols specify a validated assay before therapy begins. In July 2026, Agilent received FDA approval for PD-L1 IHC 28-8 pharmDx with OPDIVO for esophageal squamous cell carcinoma, gastric, gastroesophageal junction, and esophageal adenocarcinoma indications. This was the 8th FDA-approved indication for the platform. FoundationOne CDx also gained FDA approvals in 2026 for Itovebi and TEPMETKO. These approvals place tissue-based biomarker evaluation further into cancer treatment pathways. The cellular pathology market therefore gains demand that is linked to treatment decisions rather than discretionary laboratory spending.

High Capital and Total-Cost-of-Ownership Requirements

Digital workflows require scanners, software licenses, information technology infrastructure, image storage, and maintenance support. High-throughput scanners can process more than 300 slides per hour, but their installation also requires laboratory information system validation, network upgrades, and calibrated displays. Hospitals assess these needs as a total cost of ownership rather than as a single equipment purchase. Lower reimbursement levels can make the initial investment difficult for hospitals in Sub-Saharan Africa and parts of South America. Software and image-management systems grow as scanners are deployed. Service contracts and remote reporting arrangements also add recurring revenue. Histopathology held 42.95% of the technology-segment revenue in 2025. H&E staining and morphological review remain the standard first-line assessment for tissue-based cancer diagnosis. Immunohistochemistry remains important where treatment requires a companion diagnostic. Agilent's PD-L1 IHC 28-8 pharmDx had 8 FDA-approved indications by July 2026. Digital pathology is forecast to advance at a CAGR of 10.54% through 2031. This gives digital systems the strongest growth rate among technology categories.

Molecular pathology and AI-enabled pathology are also gaining relevance as testing becomes more biomarker-focused. Natera reported 54% year-over-year growth in oncology test volumes in the first quarter of 2026. Signatera CDx became the first FDA-approved companion diagnostic for molecular residual disease testing in oncology. Cytopathology, flow cytometry, and in situ hybridization retain specialized roles in hematological, reproductive, and other targeted workflows. Their clinical role remains important where alternative approaches are limited. Digital systems add new ways to manage and interpret cases without replacing the core value of tissue morphology.
