Acute-on-Chronic Liver Failure: Market View
Summary
This report provides a data-driven overview of the current and future competitive landscape in Acute-on-Chronic Liver Failure Therapeutics. In 2025, there were 72,095 diagnosed incident cases of ACLF in the 16 countries covered in the analyst’s epidemiology forecast.
Acute-on-Chronic Liver Failure (ACLF) management is largely supportive and focused on complications because there is no approved therapy specifically for ACLF.
Standard interventions include antimicrobials (e.g., piperacillin-tazobactam, ceftriaxone/cefotaxime, meropenem, vancomycin/linezolid), vasopressors and fluids (norepinephrine ± vasopressin), albumin/crystalloids, HBV antivirals (tenofovir or entecavir), and hepatorenal syndrome regimens (terlipressin + albumin or alternatives). Terlipressin (Terlivaz) is FDA-approved for HRS-AKI, not ACLF, while albumin products and extracorporeal liver-support devices (MARS, Prometheus) contribute to current revenues.
The consensus of PMR is that ACLF is diagnosed and graded with organ-failure scoring systems rather than validated biomarkers, and that regional frameworks (EASL‑CLIF, APASL, NACSELD) lead to meaningful differences in which patients are captured and considered treatable. In the absence of any FDA/EMA-approved disease-modifying therapy, management is primarily supportive in ICU-level care combined with treatment of the precipitating event, with escalation driven by illness severity and cause. Off-label therapies (e.g., steroids, N-acetylcysteine, terlipressin) are used on a case-by-case basis but are viewed as offering limited benefit and carrying safety risks in immunocompromised, coagulopathic patients.
KOLs highlighted that liver transplantation remains the only intervention shown to improve survival, yet only a small minority can access it. As a result, there is a substantial unmet need for treatments that can reverse organ failure and improve short-term, transplant-free survival, particularly given ~80% 28-day mortality in ACLF-3.
The ACLF pipeline holds 22 molecules, with no assets in the pre-registration stage, one asset in Phase III development, and nine assets in Phase II development.
Over the past decade, 95 clinical trials have been conducted in ACLF. The year with the most studies initiated was 2025, with 14 trials, followed by 2019, with 11 trials.
In deals involving companies developing ACLF products, North American deal activity was dominated by mergers (66.7%), with the remainder being licensing agreements (33.3%). Europe showed an equal share of licensing deals, partnerships, asset purchases, and acquisitions (25% each). Asia-Pacific activity was evenly split among acquisitions, licensing agreements, and mergers (33.3% each).
